Medical Information Document - MED-ID
The Medical Information Document - MED-ID - is a new, rather unique document that can be specifically skewed to any medical needs and requirements. The MED-ID is intended for everybody - the basic booklet is ideally suited for a healthy person; however, there are also disease-specific booklets availbale, in order to cover patients with certain chronic diseases, such as diabetes, hypertension or even cancer, and to list their continuous care, (blood) tests and treatment.
Of course, the MED-ID is available for HIV/AIDS and tuberculosis (TB) patients to support the efforts of DOTS (Directly Observed Treatment, Short course) personnel while accompanying patients along their path of treatment. Furthermore, the MED-ID is also available for children and women - in form of an overall gynecological booklet, as well as in form of an obstetrical booklet ('Mother & Child'), in order to accompany the woman during her pregnancy. The latter booklet, can also be used as a document to support PMTCT (Prevention of Mother To Child Transmission [of HIV]).
The MED-ID ia well suited as a 'Lab-Dossier' - containing not only all laboratory tests, such as blood tests, culture results and smear diagnoses, but also other diagnostic test results, such as x-rays, biopsies, sonographies, CT-scans, MRIs and more.
Advantages of the MED-ID include:
-
The ‘Medical Information Document’, is a ‘health passport’, providing disease specific information regarding the patient’s health status.
-
The MED-ID is designed in a clear format, easy to read and to complete.
-
The small format is convenient for the patient to put into his/her pocket or pouch.
-
All relevant health related information is available in disease specific booklets (e.g. HIV/AIDS, Tuberculosis, Diabetes, Hypertension, ‘OncoLog’ for cancer patients, Mother & Child, etc.).
-
The MED-ID empowers individuals to take care of their own lives and to control their own health.
-
The MED-ID reduces cost (time & money) for patients and care-providers by having all relevant health information readily available à leading ultimately to increased productivity.
-
The MED-ID records medical information originating from different physicians in one convenient document, thus it facilitates cross-clinic treatment, as well as ‘cross-country’, international treatment / follow-up on treatment.
-
The MED-ID can alleviate communication problems between patients and medical personnel, who are not familiar with the local language(s).
-
Finally, the MED-ID can serve as the base for an international health care insurance scheme and thus improve health care services to the population in an efficient and cost effective manner.
All MED-ID booklets are individually designed with your company’s logo on the cover page.
